Managing Multiple Medications

What You'll Need
You need your OHIP health card so we look up your file and process your prescription.
The name of your medication, or your old prescription bottle, or label if you're transferring from another pharmacy. This helps us get the details exactly right.
Your doctor’s name, if you know it. This can be helpful, but don’t worry if you don’t have it, we can still support you without it.
